School Forms Information

School forms may be requested by calling the clinic phone number, 651-241-5290, and selecting option 4.

You will be asked to provide the following information:

• child's name
• name of medication
• time to give medication
• dose
• address or fax number where you want us to send the forms
• your call-back number

Check that the prescription bottle is correctly labeled. If it is not, leave that information and your pharmacy phone number.
